Individual Personal Plans


The framework for the IPP’s may include but is not limited to:

  • Full needs assessment by the NASC

  • Promoting community participation.

  • The persons needs relating to personal care, household management, environmental adaptions, specialised equipment.

  • Reflecting the participation and aspirations of the person, family/whanau/advocate and other service providers.

  • Daily activities, noting friends, family and other relationship networks, medical requirements, abilities, preferences and support needs.

  • Habilitation and rehabilitation goals, and outline of any specialist supports that are in place or are achieved and time frames for completion.

  • Reviewed and developed for each person 6 monthly.

  • Support and contact information with family/whanau/advocates where appropriate allowing them to stay accurately informed about the person’s plan and changes to that plan.
