Individual Personal Plans
The framework for the IPP’s may include but is not limited to:

Full needs assessment by the NASC

Promoting community participation.

The persons needs relating to personal care, household management, environmental adaptions, specialised equipment.

Reflecting the participation and aspirations of the person, family/whanau/advocate and other service providers.

Daily activities, noting friends, family and other relationship networks, medical requirements, abilities, preferences and support needs.

Habilitation and rehabilitation goals, and outline of any specialist supports that are in place or are achieved and time frames for completion.

Reviewed and developed for each person 6 monthly.

Support and contact information with family/whanau/advocates where appropriate allowing them to stay accurately informed about the person’s plan and changes to that plan.
